Notice on Online Application for Health Declaration Certificate

Starting from November 11th 2020, foreign nationals who are to fly to China from Belgium should apply for Health Declaration Certificate online and the Embassy of China in Belgium will no longer accept application for health declaration form via email.

Foreign nationals could visit the Health Declaration Certificate system through computers or mobile phones and other terminals to fill in the information, declare the health status and upload tests reports (instructions for using the platform are detailed below). After the Chinese Embassy in Belgium has examined and approved the application, the passenger will receive an electronic Health Declaration Certificate via the online system, which is a green QR code marked with "HDC". Passengers must present the electronic or printed version of the green QR code with the "HDC" mark to the airlines upon boarding.

Annex: Operation guide of the Health Declaration Certificate system

1. Please visit the login page via https://hrhk.cs.mfa.gov.cn/H5/ using computers or scan the QR code using mobile phones (see the picture below).

2. Click on "En" in the upper right corner to switch into the English version (see the picture below).

3. After registering an account with your email address, please log in to fill in the information, declare the health status and upload the negative nucleic acid test and IgM anti-body test results (more information http://be.chineseembassy.org/eng/sghd/t1828106.htm), the information page of the passport and the itinerary.

4. The Chinese Embassy in Belgium will review the documents and issue the Health Declaration Certificate via the same online system, which will be in the form of a green QR code with an "HDC" mark.
